ACCIDENT WAIVER AND RELEASE OF LIABILITY FORM
I HEREBY ASSUME AND RELEASE ALL OF THE RISKS ON BEHALF OF MY CHILD AND MYSELF OF PARTICIPATING IN ANY/ALL ACTIVITIES ASSOCIATED WITH THIS EVENT,
including by way of example and not limitation, any risks that may arise from negligence or carelessness on the part of the persons or entities being released from dangerous or defective equipment or property owned, maintained, or controlled by them, or because of their possible liability without fault. I certify that MY CHILD is physically fit, has sufficiently prepared or trained for participation in this activity, and has not been advised to not participate by a qualified medical professional. I acknowledge that this Accident Waiver and Release of Liability Form will be used by the event holders, sponsors, and organizers of the activity in which I or My Child may participate, and it will govern my actions and responsibilities at said activity. I certify that there are no health-related reasons or problems which would prevent my child from participating in All About Sports U, All About Sports Inc. Softball Camps and events.
I Hereby agree that as Guardian my child either I or MY CHILD have not received any compensation and agree to waive any future compensation as we accept the rolls of volunteer participants.
A) I WAIVE, RELEASE, AND DISCHARGE from any and all liability, including but not limited to, liability arising from the negligence or fault of the entities or persons released, for death, disability, personal injury, property damage, property theft, or actions of any kind which may hereafter occur to me or my child from this activity,
THE FOLLOWING ENTITIES OR PERSONS:
All About Sports Inc, All About Sports U, Timothy E. Wampler, or any subcontractors of All About Sports Inc. their directors, officers, employees, volunteers, representatives, and agents, and the activity holders, sponsors, and volunteers I do hold harmless!
B) INDEMNIFY, HOLD HARMLESS, AND PROMISE NOT TO SUE the entities or persons mentioned in this paragraph from any and all liabilities or claims made as a result of participation in this activity, whether caused by the negligence of release or otherwise waive all compensation. I acknowledge that All About Sports Inc. All About Sports U, Timothy E. Wampler, and their directors, officers, volunteers, representatives, and agents are NOT responsible for the errors, omissions, acts, or failures to act of any party or entity conducting a specific activity on their behalf.
I acknowledge that this activity may involve a test of a person’s physical and mental limits and carries with it the potential for death, serious injury, and property loss. These risks are not only inherent to participants but are also present for volunteers. I hereby consent for my child to receive medical treatment which may be deemed advisable in the event of injury, accident, and/or illness during this activity. I understand while participating in this activity, I or my child may be photographed or filmed on video. I agree to allow my photos, video, or film likeness to be used for
any legitimate purpose by the activity holders, producers, sponsors, and organizers.
I CERTIFY THAT I HAVE READ THIS DOCUMENT AND I FULLY UNDERSTAND ITS CONTENT. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT AND I SIGN IT OF MY OWN FREE WILL.

Dear Parent and/or Guardian,
For good and valuable consideration, receipt of which is hereby acknowledged, you hereby irrevocably agree to allow All About Sports Inc, All About Sports U, Allaboutsportsu.com, and Timothy E. Wampler use and publication of your video and photograph(s) obtain during camps and lesson provide by said person’s and entities!!
In whole or in part on the All About Sports Inc website (www.allaboutsportsu.com) and in any other works, in any and all versions, editions, and media, and in the advertising and portion of any of them. Such videos and photograph(s) may be published, reproduced, exhibited, copyrighted, and used in and published anywhere in the world in connection with About Sports Inc, All About Sports U, Allaboutsportsu.com, Timothy E. Wampler, and in any manner whatsoever without further consent from or payment to you. You hereby irrevocably release and waive any such uses of publications, including claims relating to rights of privacy, rights of publicity, confidentiality, copyright, and any other claims, against with About Sports Inc, All About Sports U, Allaboutsportsu.com, Timothy E. Wampler its licensees, and assigns, or any other person or entity.
